We are here to help all patients who need medical care.

We ask you to help us by:

  • Only requesting home visits if you are too ill to attend surgery.
  • Whenever possible, ring before 10 am if you require a home visit.
  • Giving 48 hours notice for repeat prescriptions.
  • Cancelling your appointment if you are unable to attend.
  • Using our suggestion box if you have any comments regarding our service and would like to comment or complain, as we are happy to address any dissatisfaction that you may have as we are continually reviewing our procedures.
  • Treating our staff with courtesy and consideration. We will not tolerate any physical or verbal acts of violence or aggression and fully support the NHS policy of Zero Tolerance. Patients who are violent may be escorted off the premises and removed from our list.

YOUR RIGHTS & OURS

Protection & use of your information – Your Records & Data are safe with us!

We ask for information about you so that you can receive the best possible care and treatment. We keep this information, together with details of your care, to ensure that your doctor or nurse has accurate and up-to-date information every time we see you. The confidentiality of patient records is absolute, regardless of how the information is kept. Access to this information is very strictly controlled and your records will never be disclosed, unless you authorise it in writing.

We only use or pass information about you to people who have a genuine need for it. Whenever we can we shall remove details that identify you as an individual. Everyone working for the NHS has a legal duty to maintain the highest level of confidentiality about patient information.

Occasionally it may be advantageous to photograph a condition; in this case you will be asked to sign a consent form regarding how the photographs are used. If at any time you would like to know more about how we use your information you can speak to Jodie Billinge, Patient Services Manager. Station Drive Surgery acknowledges its responsibilities towards maintaining the health, safety and welfare of all its employees and anyone visiting the premises. We operate a zero tolerance policy and will not accept any form of violence, aggression or abuse.

We are also a Research Practice

We may use some of this information for other reasons, for example, to help us protect the health of the public generally, to plan for the future, to train staff and to carry out medical and other health research for the benefit of everyone.

We are currently involved in some research studies that require anonymous information from patients’ computerised records. You cannot be identified from this information. If anything to do with the research would involve you personally, you will be contacted to see if you are willing to take part, you will not be identified in any published results without your agreement. You have a right of access to your health records.
