Patient Online Access and Proxy Access
If you have been advised by the surgery to submit an annual review of your symptoms please use the review forms below.
Station Drive promotes the use of online access for patients. It is a quick and convenient way of ordering medication, checking test results and having access to your medical record.
The NHS App is the quickest and easiest way to obtain online access and doesn’t involve you having to come to the surgery.
As of October 2023, NHSE has granted prospective access for patients to be able to view their own medical record.
If you have a carer or family member who would benefit from having access to your online medical record, this can be arranged by contacting the GP Practice and will require your written permission.
