Dementia Link Workers

Dementia Link Workers are available to support anyone who has a diagnosis of dementia in Shropshire, Telford & Wrekin, helping them to remain socially engaged and active in their communities. This is done through telephone and face to face support and encouraging attendance to local activities.

Dementia Link Workers help contribute to the individual’s Living Plan, which will be introduced to the person with dementia at diagnosis. They will be supported to update it during regular reviews by DASS, Dementia Assessment and Support Team (formerly known as the Memory Service). Working with DASS, Dementia Link Workers provide an opportunity for people with a diagnosis to explore peer support by meeting with them at the end of the Cognitive Stimulation Therapy sessions.

Memory Cafe

The Memory Cafe runs at Ludlow Library on the 2nd Tuesday of every month from 2 to 4pm: tea, coffee, a chat and activities for anyone living with memory loss, including carers, family and friends.
